As healthcare documentation integrity professionals, we see a stark difference between a transcribed note and a provider-created note. Provider-created notes contain medical slang, misspelled or misused medical terms, undefined structure and formatting of a note, as well as other forms of abbreviated documentation. Errors we would never accept in our profession. Is our quality divided

Technology is demanding direct keyboard entry and/or front-end voice recognition entry from providers.  Does this affect patient care? Are there legal ramifications? What about CMS/Joint Commission requirements and reimbursement? How can we help bridge the “quality divide” as documentation integrity professionals?
