Documentation Errors and Risk

The EHR has developed many shortcuts to enable physicians and other healthcare providers to spend less time on documentation and more time treating patients. However, the EHR is far from perfect and documentation errors still exist, whether a transcriptionist, a voice recognition system, or a template is utilized. There are numerous legal cases where documentation errors have resulted in financial settlements. This presentation will present these legal cases as well as the responsibilities of risk management in proper training for all involved in documentation.

Learning Objectives:
1. Understand the responsibilities of risk management in preventing medical documentation errors.
2. How a template error can result in financial risks.
3. How to communicate with risk management to help prevent errors.
